Injury Claim Forms

Dear Players (and parents of Junior Players)

Although the club does carry sports injury insurance to cater for the more serious injuries that occasionally occur in local community football, the actual policy does not cover out of pocket expenses. Therefore we strongly encourage families to maintain their own private health-care policies as a back-up. It goes without saying that we highly recommend that you take out Ambulance cover in addition to your family's own private health cover. If an ambulance is called to the ground and you are not part of their membership, the cost is quite substantial.

For senior players specifically, you will note that the club does not cover loss of income if you are injured and off work for any length of time. It is encumbent on you the player to have your own personal accident insurance.

All players regardless of age, play at their own risk and acknowledge that Australian Rules football is a contact sport where injuries may occur from time to time. Should you or your child be injured during a game and you may potentially wish to raise a claim, please download the claim form below. Your Team Manager (or other Club Official) may also be required to validate the application and will need to fill in the appropriate section contained in the document.

It is considered important by club officials that you should also read the summary document below to familiarise yourself with what is and isn't covered under the actual policy itself so that you are fully aware of your own obligations and entitlements.
